How to complete a health history form within the clinic

Practice Jewel includes the ability to have clients fill out a comprehensive health history form which is then saved into your patient database.

To complete a form, follow these steps:

1. Navigate to Patient > Records
   
   
    
2. Click on the “History Forms” tab.
   
   
    
3. Click the icon for the form you wish to fill out.
   
   
    
4. Give the mouse/device to the patient and have them fill out the form itself.
   
   
    
5. Once completed, they can sign at the bottom.
   
   

Once signed, they can click save and they will be asked to return the device. You may now close the browser tab to return to the system.
